Download the Corporate Sponsorship Packet Fill out the form, save the pdf and email it as an attachment to mwill@csulbsam.org (TAX ID Available upon request, Tax deductible!)

Sponsorships and Donationsare tax-deductible since we are a non-profit organization, and are to be paid to the CSULB Foundation Building (see packet for details) (TAX ID Available upon request)

Acceptable forms of payment are: Check or Money order (completed via the instructions on page five of the corporate sponsorship packet) or using PayPal below.

STEP 3: ADJUST VIEW TO “Fit” or to your desired preference.
STEP 4: FILL OUT PAGE FIVE, ONCE DONE CLICK File, SAVE & DOWNLOAD THE PDF.
